2017 OBHG ANNUAL MEETING

FENTANYL AND MORPHINE FOR ANALGESIA IN PREHOSPITAL CARE

We have heard some passionate concerns and surprise from some Ontario Paramedics regarding the removal of fentanyl from the ACP Medical Directives and the Advanced Life Support Equipment and Medication List (Advanced Life Support Patient Care Standards (ALSPCS) version 3.3 and Provincial Equipment Standards for Ontario Ambulance Services version 2.2). We applaud this passion and encourage the dialogue on how we can collectively improve the care that paramedics provide to patients.

Given the comments we have heard, we wanted to provide an overview of the evidence-based review and decision-making that went into this decision. In summary, prehospital research of the use of morphine and fentanyl has shown the two agents have similar analgesia and similar safety profiles, but in order to improve patient safety by carrying just a single concentration of a single opioid, morphine would be the ideal single agent.
